Get It Done SB! and Mi Regalo are initiatives of Hospice of Santa Barbara focused on bringing awareness and education in English and Spanish about Advance Care Planning and its importance. Advance Care Planning (ACP) allows you to plan for the types of future medical care you wish to receive and it also allows you to select the person who will be making these decisions on your behalf. Through partnerships with a local medical institutions and community based organizations our Community ACP programs offer:
Free workshops to educate, review, notarize, and submit completed Advance Directives to Cottage Health
Knowledgeable Volunteers with specialized training to help answer questions
Targeted partnerships to provide embedded ACP services at local healthcare centers
