Patient Care Services

The Patient Care Services Program at Hospice of Santa Barbara is an interdisciplinary team approach which includes social work, spiritual care, care coordinators, and volunteer services. This program serves patients and their families impacted by life threatening illness by addressing their emotional, social, spiritual and practical concerns. The primary focus of the program is to help patients and families maintain a good quality of life.

Your Patient Care Services Team provides:

Social Work Care Management:

  • In-home evaluations and support

  • Emotional support for patients and their families

  • Coordination of care; planning for short and long term needs

  • Advocacy and connection to appropriate community resources

  • Advance care planning and assistance in navigating health care systems and services

Spiritual Care Counseling:

  • Exploration of spiritual and religious issues surrounding life threatening concerns

  • Consultation about meaning, hope, isolation, loss and death

  • Life review and reconciliation

  • Meditation, prayer, and healing energy work

  • Reconnection with faith community

  • Sacred text exploration

Volunteer Services:

Trained patient care volunteers provide social, emotional and practical support. Their activities may include: companionship, respite care, transportation, household help, errands, and shopping.
